A customer success plan template that both sides actually use

By Draft First, October 2026

A success plan is a short, shared document that says what the customer wants to achieve, how you'll get there together, and how you'll know. Most die in a folder after kickoff. The ones that work are short, written in the customer's words, and reviewed every quarter.

The template

1. Why they bought (their words)
2. Outcomes: 2 or 3, each measurable, with a target date
3. Milestones toward each outcome, with an owner on each side
4. People: sponsor, project owner, day-to-day contacts, our team
5. Risks and what we'll do about them
6. Review dates

A worked example

Harbor Point Insurance | Success plan | Agreed Oct 20

Why: "Claims tickets live in three inboxes and we miss SLAs."
Outcomes:
  A. SLA misses below 5% by March 31 (today: 18%)
  B. All claims tickets in one queue by Dec 1
Milestones:
B1. SSO live: Nov 3 | Lisa (IT), Sam (us)
B2. Inbox migration: Nov 20 | Raj (claims), Priya (us)
A1. SLA reporting dashboard: Dec 15 | Raj, Sam
People: Dana Wu (COO, sponsor), Raj Patel (owner), Lisa Grant (IT)
Risks: Dec 1 is tight; IT capacity in November [CONFIRM with Lisa]
Reviews: Jan 15 (QBR), April 15

How to make it stick

Drafting it with AI

The handoff note and kickoff notes already hold most of what you need. An agent can draft the plan from them:

From the [handoff note] and [kickoff notes], draft a one-page success
plan: why they bought in their words, 2 or 3 measurable outcomes with
target dates, milestones with an owner on each side, people and roles,
risks, and review dates. Quote the customer only from the notes. Tag
anything not clearly stated [CONFIRM]. Draft only.

Start from a solid sales to CS handoff, then run the onboarding checklist against milestone B.
